How to complete an SAQ form on Informed 365

📝 Purpose

This guide is designed to assist suppliers and vendors in completing their Supplier Assessment Questionnaire (SAQ) within the Informed 365 application. While the platform includes various form types, the process for submitting responses remains the same. If you need further assistance with your SAQ, please reach out to the client who requested your responses for guidance.

🛠️ Prerequisites

Your email address must be registered on the Informed 365 platform to access your form. Please refer to the resource titled "How to Register on an Informed 365 Application" for detailed instructions.

Before completing the form, we recommend conducting internal discussions to ensure accuracy. Additionally, please verify your responses with your legal and procurement departments before final submission.

If you have previously completed a Supplier Assessment Questionnaire (SAQ) or any other questionnaire on the Informed 365 platform, please log in using your existing credentials.

If this is your first time completing an SAQ on the Informed 365 platform, please register before logging in. For guidance on the registration process, refer to the resource titled ‘How to Self-Register on Informed 365 applications.


(a) Locating the SAQ

A Supplier Assessment Questionnaire (SAQ) may have different names within the platform, depending on its purpose. These may include:

  • Modern Slavery Supplier Assessment

  • Modern Slavery Questionnaire

  • Induction Form

  • Vendor/Supplier Checklist

  • Supplier Assessment Questionnaire

  • Responsible Sourcing SAQ

  • ...and more.

  1. Once you have registered your user email address on the Informed 365 platform and logged in, navigate to the Forms widget on your Dashboard.


  2. In this example, the SAQ is titled 'Modern Slavery Supplier Assessment'. Please ensure you select the correct form from the list.

    To open and begin completing the form, simply click on its name.


  3. The form will open in Form View, starting with the first question at the top. Please ensure you respond to all mandatory questions.

Suppliers should be mindful of the potential inclusion of new questions in a new form, even it it has been duplicated from previous years. This will sometimes necessitate additional input or responses.

The updated responses will play a key role in assessing the current risks associated with modern slavery in supply chains for participating organisations.


(b) Answering Different Question Types

  1. For questions with an open text field, click on the field to enter your response. If a numerical answer is required, enter the digits directly into the text field.

  2. When entering an address, start typing in the box with the GPS pin icon. The form will automatically suggest matching addresses in a dropdown list.

  3. When the drop down list appears, click on your street address to automatically enter it into the form.


  4. To select an Industry, click on the arrow to open the dropdown list.

  5. Check the box that corresponds to your Industry.


  6. For questions with a list of answers and checkboxes, select the box next to the applicable answer.

  7. Some forms include a 'Multiple Choice Grid' question. To complete it, select a response for each row in the grid.

    For example, if the question asks about internal policies, you must choose 'Yes,' 'No,' or 'Under Development' for each listed policy.
    A colored dot must be present in every row to proceed.

  8. For questions requiring a file upload, click on the paperclip or "Select a file..." box. This will open your file explorer. Choose the file you want to upload. Once the upload is complete, the file path will appear in the box as confirmation.


  9. All "Additional Comments" sections are optional. However, any information you provide to the client who requested your responses is invaluable.

(c) Submitting the Form

  1. If the % Complete bar at the top of the page does not reach 100%, there are still unanswered questions within the form. Click on the Unanswered Questions bell icon to highlight the questions that require responses.


  2. Unanswered questions will be highlighted within the form, making them easy to locate and complete.

  3. The form can only be submitted once it reaches 100% completion and all mandatory questions have been answered.

  4. Click Submit.

  5. After submitting the SAQ, a confirmation prompt will appear. You may take a screenshot of this confirmation for your records.

It is important to complete and submit the SAQ by the requested deadline. If you expect any delays, please inform the representative of the organisation that invited you or contact the Customer Support Team at [email protected].


(d) Checking the status of your Form

  1. Clicking on the application logo at the top left of the screen will return you to the dashboard, where you can check the status of your form.

  2. In the Forms widget, you will see a list of all forms associated with your profile, along with their current status.


  3. The Progress (%) column reflects your completion status of the SAQ, reaching 100% when all required questions are answered. Once submitted, the Progress Status will update to 'Completed,' and the Form Status will change to 'Closed,' displaying the submission date in the Submitted column.

    If you have completed SAQs in previous years, they will be listed as additional rows in the Forms section. You can view them by clicking on the corresponding row.
